1 Underweight (BMI < 18.5 kg/m2), overweight (BMI 25–29.9), and obesity (BMI ≥ 30) in non-pregnant women [52].

2 Hemoglobin concentration < 12.0 mg/dL in non-pregnant women [53].

3 Sistolic blood pressure (BP) ≥ 140 and/or diastolic BP ≥ 90 mmHg, according to the JNC 7 report [51] in non-pregnant women. Prevalence rates include women who reported current use of anti-hypertension medication as members of the group with elevated BP, independent of observed BP levels.

4 Blood glucose ≥ 200 mg/dl in non-pregnant women [54].

5 Proportions calculated with reference to women with at least one prenatal consultation with a nurse or doctor during the pregnancy with the youngest living child < 60 months of age.
