Structure of codes in Codac. Up to three codes can be assigned for each case, where the code in the first position
represents the main cause of death (COD), while the codes in the second and third
positions represent the associated conditions (AC). Within each of the three individual
codes, the first digit represents level I and the main categories in Codac (table
1), while the subsequent two digits represent the subcategories of level II and III.
