Editor’s summary
Adults with severe malaria frequently require intravenous fluid therapy to restore their circulating volume, but findings on physical examination correlate poorly with true volume status in adults with severe malaria and must be used with caution to guide fluid therapy.

Editor’s summary
In view of the growing interest in Plasmodium vivax infection, it will be necessary to better understand P. vivax prevalence and acquisition of immunity. As the parasite occurs predominantly as two alleles, the authors have made a recombinant construct based on the repeat sequences form these two alleles. This is an important accomplishment and may have broad applications to investigate naturally-acquired antibody responses to P. vivax.

Editor’s summary
Although most individuals with G6PD deficiency are asymptomatic, deficiency can result in acute haemolytic anaemia after exposure to oxidative agents, such as primaquine. A reliable test is necessary for diagnosing the deficiency to prevent an acute haemolytic crisis following treatment. This is an important paper providing a model for similar research thoughout Southeast Asia, as well as clear directions to country health planers who need to make practical decisions on G6PD screening.

Editor’s summary
Comparison of the genetic diversity of P. vivax isolates in pregnant and non-pregnant women at admission and at recurrence in a low endemic setting, and potential relevance to pathogenesis. A subtle increase in the multiplicity of P. vivax infections in pregnant patients suggests a higher susceptibility to infection.

Editor’s summary
Cerebral malaria is a devastating manifestation of Plasmodium infection and the underlying pathogenesis remains incompletely understood. This study documents that nuclear factor kappa B is one of the signaling factors that modulates apoptosis in the brain endothelial cells and intravascular leukocytes of fatal cerebral malaria.

Editor’s summary
Many insect vectors are maintained in laboratories for research purposes. Accidental release of a competent disease vector implies risk as escape into the wild can result in significant public health issues. This is possible for Aedes albopictus, for which both adult and egg stages can vector dengue viruses and several other human viruses. Image: Mating experiments with Ae. albopictus.
